Abstract


This study investigated the passive tactile thresholds (RL) of teeth, single implant and splinted multiple implant prostheses in response to both vertical and horizontal forces to clarify the effect of splinting implants to sensory function of implants. To apply reproducible forces, a solenoid-driven stimulating apparatus was used, which could be placed in contact with the test prosthesis prior to the actual force rise to avoid impact forces. From 32 patients, 6 RL levels were determined with this device and analyzed statistically. RL levels for maxillary anterior teeth to vertical and horizontal forces were 2.1 ¡¾ 0.50 g and 1.4 ¡¾ 0.45 g respectively, and showed a significant difference (P=0.00340). RL levels for single implants to vertical and horizontal forces showed a statistically significant difference (93.7 ¡¾ 40.7 g and 56.0 ¡¾ 25.4 g, P=0.0326). For the splinted implant prostheses, there was no significant difference between RL levels to vertical and horizontal forces (98.7 ¡¾ 25.0 g, 88.9 ¡¾ 23.2 g respectively). Comparison of the RLs for single and splinted prostheses indicates significant difference to horizontal forces only. That means single implant prostheses are most sensitive to lateral forces. Splinted implant prostheses still have very low RL level comparable to that of the single implant prostheses to vertical forces. Since lateral forces on implants are considered to be a potentially detrimental factor for implant success, this study may contribute to define a more favorable occlusal scheme and prosthetic designs for implant-supported prostheses.
